Job Summary
Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.
Responsibilities
- Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments.
- Reviews and updates treatment plans every 30 days.
- Performs a mini mental status exam and psychosocial assessment on all new patients.
- Performs other duties as assigned.
- Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines.
- Performs related duties as required
Required:
- Education: Master's degree in Social Work or related field.
- Experience:
- 2 years of related experience.
- Work experience may not substitute for minimum education requirement.
- Licensure/Certification:
- Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy
- CPR
