Description
Become a part of our caring community and help us put health firstHumana's Community Health Workers (CHW) serve as members of our Medicaid enrollee centric Comprehensive Care Support teams. The CHW applies a hands-on approach to enrollee engagement to support case management functions, address social determinants of health (SDOH) to improve self-management of chronic conditions, navigate the healthcare system, and promote prevention and health education that is tailored to the needs of the communities we serve. The CHW leverages their knowledge of the community and shared life experience to inform their interactions with enrollees and Humana's community partners.
Responsibilities:
- Conduct in-person assessments to understand member care needs, preferences, and socioeconomic barriers, and evaluate the home environment.
- Assist members with navigating health care and social service systems and coordinating access to basic needs (e.g., housing, food, income, transportation).
- Provide social support to help boost members' morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting member of the multi-disciplinary team.
- Support member self-management through the provision of culturally appropriate health education and health coaching

Required Qualifications
- Must reside in Illinois.
- High School Diploma or equivalent
- Minimum two (2) years prior experience working with community resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meals on Wheels etc.).
- This is a hybrid role and may require up to 25-75% daily travel to member's homes, provider organizations, Humana office in Champagne, Peoria, Bloomington, Springfield, Decatur, etc. as well as working from home office
- Valid driver's license, car insurance, and access to an automobile
- Expected working knowledge using Microsoft Office Programs specifically Teams, Excel, PowerPoint, Outlook and Word
- Must be able to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs
Preferred Qualifications
- Bachelor's degree in social work or related health field
- Community Health Worker training and/or certificate
- Familiarity with state Medicaid program guidelines
- Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy
Interview Process
As part of our hiring process, we will be using interviewing technology provided by HireVue, a third-party vendor. This technology provides our team of recruiters and hiring managers an enhanced method for decision-making.
If you are selected to move forward from your application prescreen, you will receive correspondence inviting you to participate in an SMS Text Messaging interview and/or a pre-recorded Voice Interview. You should anticipate these interviews to take approximately 10-15 minutes.
Some candidates will subsequently be informed if you will be moving forward to next round of live virtual interviews with the hiring team.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$41,900 - $56,600 per year
