Description:Job Summary:
Acts as clinical liaison between the surgery patient, family, community resources, and medical staff. Responsible for program development and ongoing oversight of the surgical care navigation services. Provides continuity of care by coordinating clinical care and services needed across the health care continuum. Manages and monitors data and key metrics related to surgical care navigation services and provides ongoing communication with health care providers. Pends orders and other tests to support plan of care for surgical patients. Develops and coordinates programs and process improvement plans relating to optimizing surgical care along the continnum. Serves as a resource to patients, families, caregivers, physicians, staff and community providers. Facilitates access to care.
Essential Responsibilities:
- Leads multidisciplinary team in planning and developing pre-op, peri-op, post-op and follow-up care/services for surgical patients. Leads and advises in development of standards, policies and procedures and educational tools for various.
- Participates in, supports, and monitors plan of care for surgical patients including referral tracking, test and conditional requirements pre and post operatively and requests for outside services and community resources. May pend orders and other tests as needed to facilitate course of care. Documents information and care provided to patients including orders, treatments, plans and instructions.
- Collaborates with providers and staff to formulate and coordinate comprehensive care plans to ensure quality care delivery. Plans and conducts professional development activities to optimize surgical care coordination. Incorporates and presents best practices and evidence-based care.
- Facilitates access to care. Integrates patients into existing programs or services. Coordinates care and services using an interdisciplinary approach. Coordinates care and services with outside agencies, in-house departments, and hospitals. Serves as liaison between internal and external care.
- Serves as a resource to patients, families, caregivers, physicians, staff and community providers. Assists patients, families and caregivers with identified problems/concerns that may impact surgical care journey. Provides appropriate information and opportunity for patients and families to discuss their wishes for their surgical care path. Educates patients, family, caregivers and staff about care and services for surgical patients.
- Manages and monitors data and key metrics related to plan of care and provides ongoing communication with health care providers. Compiles data on surgical patient care process and outcome measures as appropriate and agreed upon for type and course of surgical care. May publish and /or present data and participate in multidisciplinary team discussions and forums for surgical program performance improvement purposes.
- May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
- Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
- Incorporates the KP Nursing Vision, Model and Values throughout their Nursing Practice.
Experience
- Minimum two (2) years of direct patient care experience.
Education
- Bachelors degree in nursing OR four (4) years of experience in a directly related field.
License, Certification, Registration
- Registered Nurse License (Hawaii) required at hire
- Driver's License (from any state) required at hire
- Basic Life Support required at hire from American Heart Association
Additional Requirements:
- Demonstrated knowledge of and skill oral communication, written communication, change management, adaptability, customer service, interpersonal relations, and teamwork.
Preferred Qualifications:
- Care coordination or related experience.
- Knowledge of Community resources and available services.
- Familiarity with perioperative medications and protocols.
- Knowledge of Kaiser Health Plan benefits and system.
- Knowledge of electronic medical record system.
- Knowledge of and skill in word processing, spreadsheets and database PC applications.
- Masters degree in nursing.
