You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. For all hires in the Minneapolis or Washington, D.C. area, you will be required to work in the office a minimum of four days per week.
Primary Responsibilities:
- Make outbound calls and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Provide patient education to assist with self-management
- Make referrals to outside sources
- Provide a complete continuum of quality care through close communication with members via on-phone interaction
- Support members with condition education, medication reviews and connections to resources such as Food Banks or Transportation

Required Qualifications:
- Current, unrestricted Nevada RN license or ability to obtain a NV RN License
- 3+ years of experience in a hospital, acute care or direct care setting
- Ability to type and have the ability to navigate a Windows based environment
- Bilingual/Spanish speaking
- Background in diabetes management
- Ability to work every Saturday
- Ability to work remotely
- Ability to work independently
Preferred Qualifications:
- BSN
- Certified Diabetes Care & Education Specialist Certification (CDCES)
- Experience in a telephonic role
- Proven background in managed care

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
