Description
Optum On-Site is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team in Waupaca, WI. As a member of the Optum Behavioral Care team, you'll be an integral part of our vision to make healthcare better for everyone.As an FNP or PA, you'll provide full spectrum primary care for employees and their dependents and families of the Waupaca Foundry at the offsite Medical Clinic (location and hours below). Clinic location: N2610 WI-22, Waupaca, WI 54981
Hours: This role requires 30 on-site hours per week
Wednesdays (7a-6p CST), Thursdays (7a-6p CST) and Fridays (7a-3p CST)
Primary Responsibilities:
Deliver the highest quality primary care services including:
- Wellness and Preventative Physical Exams
- Acute care of minor illness and injuries
- Chronic Disease Management and Longitudinal care
- Minor procedures (suturing, splinting, incision/drainage, etc.)
- Lifestyle and well-being counseling
- Medical management of behavioral health conditions

Required Qualifications:
- Certified as a Family Nurse Practitioner through the ANCC/AANP or certified as a Physician Assistant through the NCCPA
- CPR certified
2+ years of experience working independently providing services in primary care, family medicine, internal medicine, ambulatory, emergency or urgent care setting
- 2+ years of experience working independently providing services in primary care or family medicine

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $109,500 to $164,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
