Description:Job Summary:
The Outpatient Infusion Pharmacist functions as an integral member of an Ambulatory Infusion Center (AIC) Team. In collaboration with physicians, nurses, other providers and support personnel providers pharmacy services to both oncology and non-oncology patients. Performs comprehensive clinical and distributive pharmaceutical services. Under direct or indirect supervision and based on patient variables such as age, sex or other physical characteristics: provides complex pharmaceutical products; develops and executes plans to manage the medication therapy of patients; performs clinical assessment and monitoring of patients; provides patient education for in-clinic outpatient chemotherapy, all pharmacologic supportive therapy. Interprets physician chemotherapy orders, conducts pretreatment patient assessment in conjunction with nurse partners, processes and dispenses medication orders in compliance with Georgia State Board of Pharmacy and federal regulations. Serves as a drug information resource for patients and providers. Maintains standardized documentation in the patient electronic medical record detailing therapeutic actions or recommendations. Prepares and dispenses injectable chemotherapy in a USP 797/800 environment; compliant setting, coordinates outpatient chemotherapy and all supportive medication needs with the main outpatient pharmacies and responds quickly to meet member/patient needs.
Essential Responsibilities:
- Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable regulations, standards, state, federal and local laws.
- Must demonstrate drug therapy monitoring and pharmaceutical care skills, including pharmacokinetic calculations, dosing recommendations, and care plan development.
- Reviews physician medication orders for appropriateness based on interpretation of data, assessments and evaluations of patient outcomes and for adherence to the formulary; clarifies orders and intervenes when necessary.
- Monitors patient drug therapy, laboratory test results for patients, confers with medical staff providers and suggests changes in drug therapy, drug utilization or therapy monitoring methods as appropriate. in the ambulatory clinic setting.
- Communicates and collaborates with patients, caregivers and members of the health care team to optimize patient care in relation to but not limited to: medication devices, proper IV line care, and the safe disposal of medications.
- Provides drug information when needed. Interacts with physicians and nurses when necessary to provide education on drug therapies or cost effective therapy and target drug utilization goals.
- Functions independently in day to day clinical, administrative and distributive duties.
- Provides direction to the pharmacy staff in resolving professional and/or clinical problems, trains employees in new procedures and techniques, monitors department staff for errors and adherence to the established department procedures.
- Orders and maintains drug, supply and equipment inventories necessary for the delivery of all admixtures. Maintains efficient inventory control and purchasing habits.
- Maintains work area to ensure storage and work areas are clean and organized for a safe work environment. Resolves drug, supply and equipment problems as needed.
- Performs biological and hazardous waste disposal.
- Participates in department quality improvement initiatives and contributes to team or departmental projects directed toward the achievement of decreased costs, improved service efficiency and patient outcomes.
- Participates in quality assurance activities, identifies potential mediation safety issues and works proactively to prevent and report medication errors and adverse drug reactions.
- Maintains records of drugs and controlled substances as required by federal and state law.
- In conjunction with Pharmacy Manager, orients and instructs new pharmacy personnel regarding departmental policies and procedures. Providers drug related training for pharmacy and other health plan employees and providers.
- Providers Pharmaceutical Care to our members including prospective, concurrent and retrospective drug utilization review, screening for drug allergies, monitors interactions and duplications, follow-up with physicians as appropriate and counseling patients.
- Demonstrate professional competency in accordance with current sterile compounding standards and participates in ACPE accredited continuing education programs. Assists in the training and education of pharmacy technicians, students, and residents.
- Partners with the Clinical Oncology Pharmacists in the medical offices at Townpark, Gwinnett and Southwood.
- Serves members in a friendly, courteous and expeditious manner, which promotes member satisfaction and retention.
- Performs general pharmacist duties including compounding and dispensing drugs and medicines; reviewing prescriptions to identify overdoses and potentially toxic compounds, bottling and packaging prepared compounds and maintaining records of legend and controlled substances.
- Participates in patient education programs and coordinates the development of educational materials as appropriate.
- Supports a collaborative Labor-Management Partnership Environment.
- Performs other duties as required.
Experience
- Minimum two (2) years of outpatient infusion pharmacy experience or home infusion experience or hospital IV pharmacy experience.
- Minimum two (2) years of admixture experience with oncology/antineoplastic agents.
Education
- Bachelors Degree in Pharmacy or Pharm.D.
License, Certification, Registration
Pharmacist License (Georgia)
- Basic Life Support
Additional Requirements:
- Possess comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies and disease states, and the ability to relate this information to the patients medical management plan.
- Excellent verbal and written communication skills.
- Demonstrated ability to extract pertinent information from the patient, or other sources and related it to the patients medical management plan.
- Experience operating pharmacy computer system(s) and other equipment.
- Demonstrated work experience doing aseptic technique and the ability to prepare sterile IV products and hazardous agents.
- Medication therapy management experience with clinic administered or infusional medications.
- Demonstrated ability to perform within an interdisciplinary environment.
- Demonstrated ability to communicate, coordinate and educate healthcare providers, patients and caregivers on infusion medications and services.
Preferred Qualifications:
- Doctors of Pharmacy, Pharm.D.
