$40,000 Student Loan Repayment or $30,000 Sign-on Bonus for individuals who have not previously participated in this program.This position is a travel position and may require up to 100% travel. Normal travel is Monday AM through Friday PM.
Optum is seeking a Nurse Practitioner - Home Based Medical Care to join our team as a Regional Delta Traveler. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.
As a member of the broader Home and Community Care team, you'll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

The DELTA team Develops Exemplary Leadership, Teamwork and Accountability and serves a critical role from team member onboarding and market implementation to ongoing education and standardization. The DELTA team member serves as a "leader-coach-player" for new market employees to provide on the job training, as well as takes a role in supporting mature markets.
Objective:
The DELTA APP is an integral member of the DELTA team and provides services to both new and mature markets. Daily activities can vary week by week based on company needs. These activities include, but are not limited to the following:
- Onboarding providers
- Assist with market engagement
- Support interdisciplinary care team (IDT) operations and weekly IDT meetings
- In office and field shadowing/ride-alongs
- Participate in interview process for new candidates
- Meet and Greets
- Emotional support to and relationship development / mentoring of new providers
- Standardization and Best Practices
The DELTA APP will work closely with the other members of the DELTA team and the Education and Learning Operations teams. The key success metrics for this position will be:
- Building a best-in-class provider team through clinical training and hands-on peer mentorship
- Implementing standardized clinical best practices in each new market
- Positive DELTA impact on KPI's
This position is a travel position and may require up to 100% travel
Training and Team building
- Leads on the job training process for new providers
- Training may also include classroom training, EMR mentorship and ride-alongs
- Completes provider competency checks as appropriate
- Serves as a peer mentor and resource for new providers, helping them learn and master the "Optum Way" of practicing
- Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
- Acts as an ambassador and role model for the Optum culture, helping to build solid, mission- driven, high-performance teams in fitting with the company mission, vision and values
Implementing Best Practices
- Establishes and maintains comprehensive understanding of Optum clinical and operational best practices
- Reinforces best practices with all team members through example, training and mentorship
Patient Care
- Become licensed and credentialed in multiple states as requested, including assisting the Optum credentialing team with timely and responsive processing of all required licensing / credentialing paperwork; Responds to licensure action items in a timely fashion
- Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing
- Performs an initial comprehensive assessment of all newly enrolled members
- Completes follow-up and post-hospital assessments according to documented standard operating procedure
- Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions
- May participate in team meetings that include, but may not be limited to:
- DELTA meetings
- Weekly IDT meetings
- Monthly staff meetings
- Continuing education/journal club programs
- All Hands Calls
Implements HEDIS measure campaigns and other quality initiatives as directed by Corporate Health Services to ensure the highest standards of care and to promote the improvement of care management and delivery- Perform related duties as necessary and other duties as assigned
Assist with market engagement
Required Qualifications:
- Completion of Nurse Practitioner accredited program
- Current state RN/NP license or ability to obtain by start date
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the ability to obtain national certification and/or NP license in state of assignment by start date
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
- Driver's license and access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy
- Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems
Preferred Qualifications:
- 5+ years of clinical experience
- Clinical training experience
Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems
**PLEASE NOTE** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
