Growing together.As a Pharmacy Care Coordinator, you will be responsible for reviewing and validating patients' medication when they are admitted to and discharged from a medical facility. The Pharmacy Care Coordinator is essential for ensuring there are no discrepancies in the medication patients are prescribed versus what they are taking. You will be working closely with pharmacists and case workers to ensure a patient's smooth transition from hospital to home. Work hours: Monday - Friday 8 AM-4 PM, MST
Location: Remote, nationwide
You will enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.
Primary Responsibilities:
- Review patient medication when they are admitted to and discharged from a medical facility
- Perform telephonic outreach for medication history consults with patients as part of the medication reconciliation process
- Work closely with pharmacists and caseworkers/social workers to confirm medication adherence and accuracy
- Access, input, and retrieve prescription information from computer systems, providers, or patients
- Other duties as assigned

Required Qualifications:
- High school Diploma/GED (or higher)
- National Pharmacy Technician Certification (PTCB)
- 3+ years of pharmacy-related experience
- Basic level of MS Office experience
Preferred Qualifications:
- Utah state Pharmacy Technician license (with ability to obtain Idaho licensure within 30 days of hire)
- Advanced Degree (i.e. Associates degree or Bachelor's degree)
- Experience in managed care pharmacy (Pharmacy Benefits Manager, Medicare Advantage)
- Experience using EMR systems
- Knowledge of Medicare part D and HIPAA
- Ability to recognize potential drug discrepancies and barriers to adherence
- Ability to identify and analyze drug data trends
- Demonstrate accuracy, timeliness, organization, and attention to detail
- Bilingual (Spanish/English)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
