Growing together.As a Community Health Worker- Housing Liaison , you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team-based structure and spend most of their time engaging directly with members telephonically. The Community Health Workeralso addresses social determinants of health such as transportation, housing, and food access.
Working Schedule: Schedule is Monday through Friday 8-5pm EST. No nights, weekends, or Holidays.
If you reside in the Northern Virginia or Tidewater, VA area, you will enjoy the flexibility to telecommute* as you take on some tough challenges.
Local travel up to 25% expected.
Primary Responsibilities:
- Complete New Member process (Welcome Call, Demographic Research, Medical Screenings, and scheduling Health Risk Assessments for members)
- Create a positive experience and relationship with the members
- Keep the members out of the hospital by supporting regular visits to their primary physician
- Keep members actively engaged with their primary physician
- Support the member to access their healthcare benefits
- Provide member education
- Support transitions of care
- Partner with care team when needed (community, providers, internal staff)
- Knowledge and continued learning of community cultures and values
- Will seek out members that have had minimal contact with the organization and try to re-establish relationships

Required Qualifications:
- High School Diploma/GED
- 1+ years of experience/knowledge of the resources available within the community (i.e, food banks, social determinants of health resources)
- 1+ years of experience with federal housing initiatives, permanent supportive housing, and/or housing first principles (i.e. HUD)
- Intermediate level of experience in Microsoft Office applications such as Excel (create, edit, sort), Word (create & edit), and Outlook (calendar management). Candidates must have the ability to create, edit, copy, send and save documents, correspondence and spreadsheets
- Access to high-speed internet from home / Broadband cable or DSL
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Must be willing to work core business hours with overtime as needed
- Reside in the state of Virginia
Preferred Qualifications:
- Certified Nursing Assistant, Home Health Aide or Medical Assistant, or LPN
- Community Health Worker (CHW) accredited
- Experience working in Managed Care
- Experience in productivity-driven positions
- Knowledge of VA Medicaid population
- Bilingual in Spanish and English

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $20 to $36 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
