Become a part of our caring communityThe Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements. We do this to achieve and/or maintain an optimal wellness state. You will guide members/families toward resources and facilitate interaction with them. These resources are appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Field Care Manager, Behavioral Health assesses and evaluates member's needs and requirements. This is done to achieve and/or maintain an optimal wellness state. The manager guides members/families toward and facilitates interaction with resources appropriate for the care and wellbeing of the member.
Position Responsibilities
The Field Care Manager, Behavioral Health (Care Manager, Behavioral Health) employs a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues. Identify and resolve barriers that hinder effective care.
- Perform telephonic and face-to-face assessments and evaluations of the member's needs and requirements. These assessments aim to achieve and/or maintain an optimal wellness state by guiding members/families toward the appropriate resources for the care and overall wellbeing of the member.
- Ensure member is progressing towards desired outcomes by continuously monitoring care through assessments and/or evaluations.
- Create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas.
- Collaborate with providers and community services to promote quality and cost-effective outcomes.
- Deliver needed services/supports for Behavioral Health, Physical Health, Social Determinant of Health and value added benefits.
- Coordinate across the transdisciplinary care team (at a minimum the PCP) and transitions of care
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences
- Submit incident reports

Required Qualifications
- 3+ years experience as LPC, LCSW, LAC or psychiatric RN
- Applicable State licensure in field of study
- Intermediate to Advanced computer skills and experience with Microsoft Office
Preferred Qualifications
- Bilingual a plus
- Previous managed care experience
- Certification in Case Management
- Experience working with children and adolescence
Additional Information
Workstyle: Combination remote work at home and onsite/home member visits
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$65,000 - $88,600 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
