Job Summary
This position may qualify for a sign-on bonus.
Responsible for coordinating clinically complex patients care across a continuum; ensuring and facilitating the achievement of quality clinical and cost outcomes; negotiating, procuring and coordinating appropriate services and resources needed by the patient; and at key points, intervening to address and resolve issues/concerns. This individual is responsible for coordinating the clinical plan of care to conform to the evidence-based practice and regulatory requirements. This position integrates care coordination utilization management, and discharge planning.
Responsibilities
Patient care employees must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups-must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs in relation to age and to provide the care needed as described in the department's policies and procedures.
Qualifications
Minimum Education
Required qualifications should include
- RN (minimum of B.S.N. preferred) with at least 5 year's beside experience or a LPN/LVN with 10 years in the relevant clinical specialty (e.g. , case manager for pediatrics patients should have pediatric nursing experience,
- excellent organizational skills,
- excellent verbal and written communication skills;
- ability to lead and coordinate activities of a diverse group of people;
- demonstrated critical thinking and problem solving skills; and
- computer literate

Organization Description
Nacogdoches Medical Center has been serving the East Texas community for nearly 50 yearswith a focus on caring for patients and a strong team environment. Service lines at the hospital include obstetrics and gynecology, cardiology (including a cath lab and electrophysiology capabilities), cancer care, neuroscience (Brainlab technology and stroke treatments) and weight loss surgery.
