Job Summary
The Patient Navigator will assists in the intake process from initial patient contact to any follow-up requirements. This
position will manage each account to improve the patient experience. Identifies patients with specific diagnoses and
connects those patients to coordinated care management. Educates patiens on care management options in accordance
with policies and procedures.
Responsibilities
- Performs duties and conducts interpersonal relationships in a manner designed to project a positive image of the department.
- Works with Administrator/Physician and other members of section or department to promote a harmonious work environment.
- Ensures that all contacts with patients, the public, physicians and other personnel are carried out in a friendly, courteous,
helpful and considerate manner. - Answers telephones in a prompt and courteous manner.
- Displays concern and provides assistance or explains procedures as appropriate to callers or in face-to-face situations.
- Dedicated to meeting the expectations and requirements of internal and external customers.
- Works with customers to identify problem areas and recommend solutions.
- Maintains patient confidentiality and adheres to HIPAA laws.
Qualifications
Education
Bachelor of Science Degree
Experience
Experience in patient process, service related field, marketing products & services
Strong verbal & oral communication skills
Experience in EMR - NextGen system a plus
Marketing experience preferred
