Reliant Medical Group, part of the Optum family of businesses, is seeking a Full Time Primary Care (IM and FM) New Patient Intake Clinician (NP or PA), 1.0 FTE to join our team in Worcester , MA. Position Highlights & Primary Responsibilities:
- Primary Care (IM and FM) Intake / Access APC role focused on improving access for new Primary Care patients and supporting practice growth
- Serve as the first clinical point of contact for patients entering the Adult and Family Medicine practice
- Focus on new patient visits, problem-focused evaluations, and care navigation, with appropriate transition to long-term primary care providers
- Clinical responsibilities include acute and subacute concerns, chronic disease assessment and stabilization, and initiation of preventive care
- No procedural responsibilities beyond standard outpatient IM/FM scope
- Anticipated patient volume of 14-18 patients per day, based on visit type and complexity
- 20-30-minute visit lengths, with flexibility for more complex visits
- Highly collaborative, team-based environment working closely with IM physicians, APCs, nursing staff, and medical assistants
- Key access role supporting timely patient evaluation and panel growth
- Monday-Friday, daytime schedule
- No call, weekend, or holiday responsibilities
- Well-suited for NPs or PAs who enjoy access-focused care, care coordination, and team-based primary care

Required Qualifications:
- Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Unlimited Licensure in the state of Massachusetts
- Active DEA License
- Min of 3yr experience as a APC
- Experience working within a Team Care model
- Fluency in EMR systems & technology
- EPIC experience a plus
