Job Summary

Job type: Full Time
POSITION SUMMARY:
Responsible for monitoring the delivery of care to patients identified by case finding criteria and/or referral for case management services. Identifies variances and assists in the development of strategies to reduce variance patterns. Ensures appropriate utilization of facilities and services and provides for the review of admission, continued stay and professional services rendered. Coordinates discharge plan and needed social service and/or chaplain services. Ensures planning to facilitate patient's return to their optimum level of function. Performs data collection as indicated for Commitment to Quality. Performs other duties as required.
Qualifications
Education:
Associate degree or equivalent
License/Certifications:
Valid California RN license and Healthcare Provider BLS.
Experience:
3 years (acute care experience)
